AUTHORIZATION AND RELEASE: General terms: Fantastic Fido’s will exercise reasonable care for the safety of your pet and to keep the boarding premises safe and secure. Boarding pets will be fed twice daily. Fresh water will be available at all times. Fantastic Fido’s cannot guarantee against accidents. We will not be liable for loss or damages to pet’s belongings. Only clean, freshly-laundered items will be accepted into the facility with boarded pets.*

Vaccination/Flea & Tick Prevention: All animals must be current on yearly vaccine for Rabies, DHLPP and every 6 months for Bordatella (kennel cough preventative.) To avoid a $15 fee, the owner must provide to Fantastic Fido’s the veterinarian records on paper for these vaccines at least 48 hours prior to boarding or daycare. Records may be faxed. All animals accepted for Boarding or Daycare must be on monthly flea and tick prevention such as Frontline or Advantage or equally high quality products. I agree to hereby administer these products AT LEAST 48 hours prior to boarding at Fantastic Fido's.*

Dogs six months of age or older must be spayed or neutered to be accepted for boarding, daycare or training. Deposit Requirements/Cancellation Policy: All clients are required to provide a credit card number to make boarding reservations. Cancellations or early departures received with less than 48 hours notice will be charged 2 days of boarding.*

Boarding Requirements: Pets are to come in clean, not matted, and nails clipped, with nylon or leather collars (no choke collars or harnesses.) Owners are to provide animals usual food. Do not include food the animal is not accustomed to eating. Dry food is to be in an airtight container. Soft food should be in pouches or pull-top cans if possible. Cat owners must provide litter and litterbox. If pet is trained to use training pads, these must be provided by owners. Owners may also bring treats, toys and bedding. All items brought in must be labled with your pet’s name. Do not bring your pet’s dishes.*

Medication and Treatment Authorization: Prescription medications must be provided in the original container with the veterinary’s instructions and will be given at no extra charge. Over-the-counter meds may be given at a $3 per day charge. Fantastic Fido’s is authorized by the owner to seek veterinary advice and care, including emergency care, at the owner’s expense. We will attempt to contact you before obtaining that care, but by agreeing below, this document serves as full authorization to obtain veterinary care for your pet at your expense, whether or not you have been contacted. Please note that we DO NOT offer a “Do Not Resuscitate” option for any pet staying with us.*

Daycare and Group Play necessarily involve dogs interacting and playing together. Although these activities are supervised, there is a possibility of injury from rough play or combative behavior. There is also an increased risk of transmission of coughs, colds and parasites. Outdoor play may include getting wet in the wading pools in appropriate weather. By checking that you agree below, you acknowledge that you are aware of the risks involved with any canine group activities and that you agree to hold Fantastic Fido’s harmless for these risks and that you are authorizing your dog to enter into our group play or daycare program at our discretion.*
